Treatment Consent & Liability Waiver

This consent and waiver applies to the spa, massage, facial, and nail services you receive with us. It takes effect once BOTH you and your practitioner have signed below. You may sign in-app, or print, sign, and upload a scanned copy. Please read each section and tick the boxes to confirm you understand.

Section 2 — Informed Consent

I consent to receive the treatment(s) named above and understand what they involve, including disrobing, draping, and direct skin contact where relevant.

I understand that spa treatments are for relaxation and wellbeing and are not a substitute for medical diagnosis, treatment, or advice.

I will tell my practitioner immediately if any pressure, temperature, product, or technique causes discomfort or an adverse reaction.

Section 3 — Contraindication Acknowledgement

I have disclosed all health conditions, injuries, medications, pregnancy, and allergies I am aware of, and understand that withholding information may make a treatment unsafe.

I understand that a patch test may be recommended before certain products or treatments, and that declining one is at my own risk.

Section 4 — Photo Use Consent

I consent to before-and-after photographs being kept in my confidential client record only.

I separately and optionally consent to my anonymised photographs being used for the spa’s marketing or social media.

Section 5 — Cancellation & Refund Terms

I have read and accept the cancellation and refund terms above.

Section 6 — Liability Waiver & Acknowledgements

I release the spa and its practitioners from liability for any reaction, discomfort, or injury arising from information I did not disclose or from pre-existing conditions.

I have read and understood each section of this agreement and have had the opportunity to ask questions.

I consent to electronic signatures having the same legal effect as handwritten signatures.

I confirm I am 18 or older, or have the consent of a parent or guardian who is co-signing.
