This is the studio’s standing file for one student — a single copy the teacher and the family keep current for as long as the student studies here. It is never re-filled per term. If a substitute covers a lesson, this is the page they read first.
Student & Contact Details
Guardian name (if the student is a minor):
Emergency contact and phone:
Record every instrument the student plays here, including rentals. Keep serial numbers current — they matter for insurance and for returning a rental at the end of the year.
Current Teaching Arrangement
Regular lesson day and time:
Teacher changes over time — who taught the student, from when to when, and why the change:
One line per term. Add a new row each term rather than editing the last one — the shape of the progression is the useful part.
Cumulative — add pieces as they are finished, never clear the list. Note composer, arrangement and roughly when it was completed.
Exams, Competitions & Performances
Next exam or entry deadline:
Tick each milestone once it is secure, not merely attempted.
Health, Access & Practical Notes
Hand, wrist, back or hearing issues; anything that affects how long the student can play; learning or sensory needs; access requirements in the building:
What works in a lesson — pacing, motivation, how the student responds to correction, anything a substitute teacher should know:
Where the student and family want this to go — exam track, conservatoire audition, playing in a band, or simply playing for pleasure. Revisit this once a year:
Dated entries as things change — a new instrument, a slot moved, a plateau, a breakthrough, a conversation with the family. Newest at the top.
Attach exam certificates, rental paperwork or recital recordings:
Keep this record current — it travels with the student through every teacher, every grade and every recital.
