Get started with PocperThis agreement sets out the terms of a 12-month preventive-care wellness plan between the veterinary clinic ("Clinic") and the pet owner ("Owner"). It becomes effective only when BOTH parties have signed below. The Clinic completes the plan coverage and fee tables, then shares this document with the Owner for review and countersignature — either in-app, or by printing, signing, and uploading a scanned copy.

The plan fee is a monthly instalment covering the full 12-month term. It is payable each month regardless of how many covered services are used, in exchange for spreading the cost of preventive care across the year.

Covered services not used during the plan term expire at the end of the term and do not roll over or convert to cash, credit, or other services. The plan is a preventive-care package, not an insurance policy, and does not cover illness, injury, emergency, or surgical treatment.
I understand unused services do not roll over and that this plan is not pet insurance.
If the Owner cancels before the end of the term after using covered services worth more than the fees paid to date, the Owner agrees to pay the difference between the Clinic’s standard price for services already received and the plan fees already paid. Either party may terminate for material breach that remains uncured 14 days after written notice.

At the end of the term the plan renews automatically for a further 12 months at the Clinic’s then-current rates unless either party gives written notice of non-renewal at least 30 days before the term ends. The Owner authorises the Clinic to keep medical records for the pet and to contact the Owner with reminders for due preventive care.
