Section 4 — Consent & Agreement

By submitting this form, I confirm and agree to the following:

1. I confirm that the health information provided is accurate and complete. I understand that withholding information may affect the safety and outcome of the grooming service.
2. I understand that grooming involves potential risks for elderly, medically compromised, or highly anxious pets, and that the groomer will act with the utmost care. I will not hold the salon liable for pre-existing conditions that become apparent during grooming.
3. I understand that severely matted coats may require a shave-down for the comfort and welfare of my pet, and I consent to this if deemed necessary by the groomer.
4. I agree to collect my pet promptly when notified that the service is complete. A waiting fee may apply for delays of more than 30 minutes.
5. I agree to the quoted service price and understand that additional charges may apply if unexpected conditions (e.g. matting, parasites) are discovered during the appointment.
