Annual Grooming Membership Agreement

This agreement sets out the terms of an annual grooming membership between the grooming salon ("Salon") and the pet owner ("Member"). It becomes effective only when BOTH parties have signed below. The Salon completes the membership and pricing details and signs first, then shares this document with the Member for review and countersignature — either in-app, or by printing, signing, and uploading a scanned copy.

Section 2 — Included Grooming Services

During the term, the membership includes the following grooming services on the schedule shown:

Services not listed above, or extra add-ons requested on the day, are billed separately at the Salon's standard rates.

Section 3 — Membership Fee & Payment

The membership fee is charged at the start of each billing cycle and is due within 14 days. The fee is payable whether or not every included session is used, in exchange for the Salon reserving regular grooming slots for the Member's pet.

Section 4 — Scheduling, Missed Visits & Cancellations

Included sessions must be booked in advance and used within the term; unused sessions expire at the end of the term and do not roll over or convert to cash or credit. The Member agrees to give at least 24 hours' notice to change or cancel an appointment. Missed appointments or late cancellations may count as a used session or incur the Salon's no-show fee.

Section 5 — Pet Health, Safety & Consent

The Member confirms that the pet's vaccinations are current and agrees to disclose any medical conditions, allergies, or behavioral concerns before each visit. The Salon will always prioritize the pet's comfort and safety and may stop or reschedule a groom if the pet becomes distressed, is aggressive, or has a health issue. In a genuine emergency the Salon may seek veterinary care, and the Member is responsible for the resulting costs.

I consent to the Salon seeking emergency veterinary care for my pet if needed, at my expense.

Section 6 — Photos & Renewal

After the term, this membership renews automatically for successive 12-month periods unless either party gives written notice of non-renewal at least 30 days before the term ends. The Salon may take before-and-after grooming photos for the Member's records and, only with the Member's permission below, may share them on the Salon's social media or website.

Section 7 — General Acknowledgments

I have read and understood each section of this agreement.

I have had the opportunity to ask questions and seek independent advice.

I consent to electronic signatures having the same legal effect as handwritten signatures.
