Annual Spa Membership Agreement

This agreement sets out the terms of an annual wellness membership between the spa ("Provider") and the member ("Member"). It becomes effective only when BOTH parties have signed below. The Provider completes the membership tier, benefit, and fee details, then shares this document with the Member for review and countersignature — either in-app, or by printing, signing, and uploading a scanned copy.

Section 2 — Membership Tier & Monthly Benefits

Each month of the term, the membership includes the benefits recorded below:

Member benefits are personal to the Member and may not be shared, sold, or transferred unless the Provider agrees in writing. Any service not listed above is charged at the Member rate in Section 3.

Section 3 — Fees & Payment

The monthly fee is charged in advance on the same day each month and is payable whether or not the Member uses their benefits that month, in exchange for the Provider reserving capacity for the Member. A payment more than 7 days late may pause benefits until the account is settled.

Section 4 — Unused Benefits & Rollover

Unused monthly benefits expire at the end of each month and do not roll over unless the Provider agrees otherwise in writing before the month ends. Benefits have no cash value and may not be exchanged for cash, credit, or retail products.

Section 5 — Bookings, Cancellations & No-Shows

Appointments are subject to availability and should be booked in advance. A cancellation or reschedule made less than 24 hours before the appointment, or a no-show, uses that month’s benefit or is billed at the Member rate. Please arrive on time; late arrivals may have their treatment shortened so the next guest is not delayed.

Section 6 — Renewal, Freeze & Cancellation

After the initial 12-month term, the membership renews automatically month to month unless either party gives written notice of non-renewal at least 30 days before the term ends. The Member may request to freeze the membership for up to two consecutive months per year for medical or travel reasons, on written notice. Either party may cancel for material breach that remains uncured 14 days after written notice. On cancellation, fees already paid for the current cycle are non-refundable and any joining fee is non-refundable.

Section 7 — Health, Consent & Liability

The Member agrees to disclose relevant health conditions before each treatment and to follow the therapist’s aftercare guidance. Spa treatments are for relaxation and wellbeing and are not a substitute for medical care. The Provider is not liable for reactions arising from information the Member did not disclose. Section 8 — General Acknowledgments

I consent to electronic signatures having the same legal effect as handwritten signatures.
