Get started with PocperThis agreement sets out the terms of an ongoing home maintenance plan between the service company ("Provider") and the homeowner ("Client"). It becomes effective only when BOTH parties have signed below. The Provider drafts and signs first, then shares this document with the Client for review and countersignature — either in-app, or by printing, signing, and uploading a scanned copy.
Provider (company name):

Client (full legal name):

Initial term:
During the term, the Provider will perform scheduled maintenance on the following systems:
System / area | Visits per year | Scope of each visit |
|---|---|---|
Any work not listed in the table above is out of scope and handled under Section 4 (Repairs & Out-of-Scope Work).
I have reviewed the covered systems and visit schedule and agree to this section.
Item | Amount | Billing cycle |
|---|---|---|
Maintenance plan fee | ||
Member discount on repairs | ||
Emergency call-out rate (after hours) | ||
Late payment interest |
The plan fee is payable in advance for each billing cycle and is due within 14 days of invoice. The fee is payable regardless of whether all scheduled visits are used, in exchange for the Provider reserving priority capacity for the Client.
I have read and agree to the fee and payment terms in this section.
Scheduled maintenance covers inspection, cleaning, adjustment, and minor consumables only. Parts, repairs, or replacements identified during a visit are out of scope and require a separate written estimate approved by the Client before work begins. Plan members receive the discount shown in Section 3 on approved repairs. The Provider is not responsible for pre-existing defects or damage caused by others.
Client notes on known issues or systems needing special attention:
I understand that repairs and out-of-scope work require a separate approved estimate.
The Client will provide safe, reasonable access to the property for scheduled visits and will keep work areas reachable. The Provider will give advance notice of each visit and reschedule if the Client is unavailable. Visits missed without at least 24 hours notice may be counted as used or subject to a call-out charge.
I agree to provide access and understand the missed-visit policy.
After the initial term, this agreement renews automatically for successive periods of the same length unless either party gives written notice of non-renewal at least 30 days before the current period ends. Either party may terminate for material breach that remains uncured 14 days after written notice. On termination, the Client pays for all visits performed and capacity reserved through the effective termination date.
I have read and agree to the term, renewal, and termination provisions.
I have read and understood each section of this agreement.
I have had the opportunity to ask questions and seek independent advice.
I consent to electronic signatures having the same legal effect as handwritten signatures.
I am authorized to enter into this agreement on behalf of the party named above.
