Annual Skincare Membership Agreement

This agreement sets out the terms of an annual skincare membership between the aesthetic clinic ("Provider") and the member ("Member"). It becomes effective only when BOTH parties have signed below. The Provider drafts and signs first, then shares this document with the Member for review and countersignature — either in-app, or by printing, signing, and uploading a scanned copy.

Section 2 — Included Treatments & Benefits

During the term, the membership includes the following treatments and benefits:

Treatments not listed in the table above are not part of the membership and are quoted and billed separately at the Provider's standard prices.

Section 3 — Membership Fee & Payment

The membership fee is payable in advance according to the billing cycle above and is not refundable once a membership year has started, except where required by applicable consumer law. The fee is payable regardless of how many included treatments the Member actually uses, in exchange for the Provider reserving capacity and member pricing for the Member.

Section 4 — Scheduling, Cancellations & No-Shows

Included treatments are provided by appointment, subject to availability. The Member may reschedule or cancel an appointment free of charge with at least 24 hours' notice. Appointments cancelled with less notice, or missed without notice, may be counted against the Member's included allowance for that year. Repeated no-shows may result in the Provider requiring pre-booked confirmation for future appointments.

Section 5 — Medical Suitability & Safety

Every included treatment remains subject to a clinical suitability check on the day. The Provider may postpone or substitute a treatment if, in the treating practitioner's professional judgment, it is not safe or appropriate for the Member at that time (for example due to pregnancy, active skin infection, recent medication changes, or new contraindications). The Member agrees to keep the Provider informed of any changes to their health, medications, or allergies, and to complete a separate informed-consent form before each procedure that requires one. Postponed treatments are rescheduled and do not reduce the Member's included allowance.

Section 6 — Unused Treatments & Non-Transferability

Included treatments expire at the end of each membership year and do not roll over, unless the Provider agrees otherwise in writing before the year ends. The membership and its included treatments are personal to the Member and may not be transferred, shared, resold, or exchanged for cash, credit, products, or treatments outside the scope in Section 2.

I understand that unused treatments expire at the end of each membership year and that my membership is non-transferable.

Section 7 — Termination & Renewal

After the initial term, this agreement renews automatically for successive 12-month periods at the then-current membership fee unless either party gives written notice of non-renewal at least 30 days before the current period ends. Either party may terminate for material breach that remains uncured 14 days after written notice. If the Provider permanently ceases to offer the membership, the Member receives a pro-rated refund of the fee for the remaining unused portion of the current year. On termination for the Member's breach, treatments already used are charged at standard (non-member) prices and offset against any refund due.
